However, following major policy shifts within Department of Health and Human Services in April 2026, the FDA scheduled a public hearing before its Pharmacy Compounding Advisory [01:07] Committee, or PCAC. Over 2 days of expert testimony, the advisory panel decided to override the official recommendations of agency staff and voted in favor of six out of seven evaluated peptides. [01:22] Today, we're going to break down exactly what happened at this meeting, explain what the different regulatory categories mean for compounding, examine the specific peptides evaluated, and explore the crucial next steps for patients and [01:35] clinicians navigating this evolving landscape. To understand the significance of this meeting, we first need to understand how the FDA regulates compounding pharmacies under section 503A of the Federal Food, [01:50] Drug, and Cosmetic Act. Traditional compounding pharmacies prepare customized medications prescribed by licensed physicians for individual patients whose medical needs cannot be met by commercially [02:02] cannot be met by commercially manufactured FDA-approved drugs. Under section 503A, compounding pharmacies are permitted to use bulk drug substances, meaning the raw, pure active pharmaceutical ingredients, if [02:17] those substances appear on an official codified list known as the bulk drug substances positive list, or if they are components of approved medications, or have a recognized United States Pharmacopoeia monograph. [02:33] Because the formal process of adding a substance to this positive list can take years, the FDA established an interim policy dividing nominated bulk categories. Category one includes substances [02:48] nominated with sufficient scientific and clinical information that are currently under formal evaluation. Substances in category one are permitted for compounding under the agency's informal enforcement discretion while [03:01] the evaluation is underway. Category two consists of substances that the FDA believes raise significant safety concerns. When a substance is placed in category two, it is effectively banned from [03:14] compounding, and pharmacies that prepare it face direct regulatory enforcement. Category three includes substances nominated without adequate scientific data, which are also not permitted for compounding. [04:12] On July 23 and 24, 2026, the PCAC convened to formally evaluate seven nominated peptides for compounding under section 503A. [04:24] Going into the hearing, agency staff scientists presented formal briefing documents recommending that the committee vote against all seven peptides, citing a lack of traditional randomized double-blind phase three [04:36] clinical trials demonstrating safety and efficacy. However, the independent committee members, composed of practicing physicians, clinical pharmacologists, compounding pharmacists, and patient [04:49] advocates, took a different view. After reviewing extensive clinical data, published preclinical research, and hearing hours of public testimony from clinicians and patients, the panel voted, often by a narrow margin, to [05:02] recommend six of the seven evaluated peptides.
